Abstract

Kepatuhan manajemen nutrisi harian menjadi pilar utama dalam mengendalikan kadar gula darah dan mencegah komplikasi kronis pada penderita Diabetes Melitus (DM). Namun, rendahnya pemahaman mengenai pembatasan porsi makan sering kali menjadi hambatan utama di tingkat komunitas. Kegiatan Pengabdian kepada Masyarakat (PkM) ini bertujuan untuk meningkatkan kesadaran, pengetahuan, dan keterampilan psikomotorik penderita DM di Desa Pasar Jati melalui intervensi edukasi pola makan berbasis prinsip 3J (Jadwal, Jenis, Jumlah). Metode yang digunakan meliputi skrining klinis kadar gula darah harian pada 25 responden, intervensi edukasi menggunakan Modul 3J, demonstrasi porsi makan sehat dengan metode "Isi Piringku" menggunakan bahan makanan mentah riil, serta pemantauan keberlanjutan melalui kunjungan rumah (home visit). Hasil evaluasi menunjukkan adanya pergeseran kognitif kolektif yang signifikan, di mana akumulasi skor jawaban benar meningkat dari 74,08% saat pre-test menjadi 86,24% saat post-test. Secara individu, 76% responden mengalami peningkatan nilai mandiri, dan kelompok dengan tingkat pengetahuan kategori "Kurang" berhasil ditekan hingga 0%. Kesimpulan dari kegiatan ini menunjukkan bahwa intervensi komunikasi kesehatan yang terstruktur dan aplikatif terbukti efektif meningkatkan kemandirian penderita DM dalam mengontrol pola konsumsi di lingkungan rumah tangga secara berkelanjutan.
